Located along a scenic waterfront property in Brooklyn, NY, our Menorah Center for Rehabilitation and Nursing Care is seeking a Physical Therapist to evaluate, assess and treat patients within our rehabilitation center.
Evaluate residents to assess appropriate Physical Therapy treatment and rehabilitation potential
- Establish Appropriate Treatment goals and Plan of treatment for each resident.
- Plan and administers treatment modalities in accordance with a resident plan of care
- Documents screenings, evaluations, progress notes and care plan tracking forms
- Maintains statistical data on patients
- Provides guidance and supervision to assigned Physical Therapy Assistants
- Bachelor's degree in physical therapy
- Current NYS Physical Therapy license
- 2 years of geriatric experience from a rehabilitation & nursing home facility is preferred
