Physical Therapist (PT) – Full Time | Skilled Nursing Facility

We are seeking a compassionate and motivated Physical Therapist to join our Skilled Nursing Facility team. This role is ideal for a clinician who thrives in a collaborative, patient-centered environment and is passionate about helping older adults regain function, maintain mobility, and improve their quality of life.
Key Responsibilities:
- Perform comprehensive evaluations for residents with orthopedic, neurological, cardiopulmonary, and medically complex conditions
- Develop individualized treatment plans tailored to resident goals, functional needs, and SNF regulatory guidelines
- Provide therapeutic interventions including strengthening, balance training, gait training, transfer training, pain management, and functional mobility retraining
- Collaborate closely with nursing, occupational therapy, speech therapy, and social services to support interdisciplinary care
- Document evaluations, daily notes, progress reports, and discharge summaries in accordance with Medicare, state, and facility requirements
- Educate residents and caregivers on safe mobility, fall prevention, and home exercise programs
- Maintain a safe and supportive therapy environment
Qualifications
- Active state Physical Therapist license
- Excellent communication, documentation, and clinical reasoning skills
- Experience in geriatrics or skilled nursing strongly preferred, but new graduates with a passion for older adults are welcome
