Reviews and interprets medication orders written by physicians or other authorized health care providers.
Verifies orders for completeness, accuracy and appropriateness in accordance with pharmaceutical care standards and prescribed policies and procedures.
Reviews patient profiles and problems, i.e. drug interactions, drug allergies, therapeutic duplication, contraindications, drug-drug incompatibilities, dosing, and adverse drug reactions relating to the new order or order changes.
Maintains effective communication with physicians and nurses to ensure safe and appropriate use of drugs.
Performs clinical duties including but not limited to medication counseling, medication reconciliation, pharmacokinetic monitoring, and renal dose monitoring.
Prepares or supervises preparation and accurate dispensing of medication to inpatients and outpatients, including unit dose dispensing, IV admixture, chemotherapy, TPN, investigational drugs, other sterile products, and extemporaneous preparations, in accordance with hospital policy.
Completes written records of dispensing for all medication including controlled substances and investigational drugs as required by policy and regulation.
Advises and consults with patient and families on medication use, dangers, side effects and composition.
Maintains responsibility for resolving medication dispensing system problems for one or more patient care areas. Provides in-service education and/or orientation concerning drug use to health professionals when necessary or as appropriate.
Attends and participates in meetings held on patient care units as appropriate.
Routinely visits Nursing units, reviews patient charts, physician orders, medication records and storage areas on unit.
Conducts monthly inspections of patient care areas in accordance with Joint Commission N.Y.S. Department of Health and CMS requirements and standards.
Reviews inspection results and quality improvement information with Patient Care Director or department head and recommends changes as required.
Routinely participates in target drug monitoring programs and pharmacy and hospital sponsored drug therapy intervention programs.
Participates in quality assurance, drug utilization review, and adverse drug reaction reporting as needed.
Participates in medical work rounds when appropriate and necessary.
Maintains knowledge of field by participating in continuing education programs in accordance with department policies as stated in the Joint Commission standards and as necessary to perform the job effectively.
Documents clinical interventions.
Accepts the responsibility of being in charge of pharmacy satellite activities.
Adjusts the workflow activities for pharmacy interns, technicians, students, other staff members, and volunteers to assure that all work is completed when necessary.
Notifies a pharmacy supervisor or administrator regarding problems beyond the scope of his/her responsibilities.
Oversees and participates in maintaining and properly storing an adequate inventory of pharmacy items.
Assures stock is rotated and expiration dates are checked.
Obtains items not stocked or out of stock from authorized sources according to established procedures.
Participates in training and continued competency activities of other pharmacists, pharmacy Interns, technicians and students.
Contributes to new employee orientation.
Provides input to Pharmacy
Supervisor concerning performance issues.
Provides in-service training to other healthcare staff.
Participates in staff meetings or hospital interdisciplinary meetings, recommends changes in policy and procedures where necessary to improve operations and quality of service.
