Position Summary:
The Patient Navigator supports individuals receiving care provided by Eastchester Family Services at the MUST Community Clinic by helping patients access affordable healthcare services. This role assists uninsured and underinsured patients in navigating financial eligibility, completing required documentation, and coordinating referrals for specialty care, imaging, and laboratory services and coordinating access to prescription medications for eligible patients through the Dispensary of Hope program. The Patient Navigator plays a key role in reducing barriers to care and improving health outcomes for vulnerable populations.
The Patient Navigator will start as a part-time position working three full days (Monday, Tuesday, and Wednesday) from 9:00 am - 5:00 pm daily. As the clinic schedule expands to include four/five days a week, so will the Patient Navigator position.
Key Responsibilities:
- Meet with patients to assess financial eligibility for free or reduced-fee services needed that extend beyond services provided by MUST’s medical care provider (Eastchester Family Services) serving patients at the Community Clinic
- Assist patients in completing applications for financial assistance programs and specialty care referrals
- Collect and review income documentation and personal information to determine qualification for services
- Coordinate referrals to specialty providers, imaging centers, and laboratory services
- Educate patients on available healthcare resources and services within the community
- Maintain accurate, confidential patient records in compliance with HIPAA regulations
- Collaborate with clinic staff, including providers from Eastchester Family Services, and the referral provider to ensure continuity of care
- Follow up with patients regarding application status, appointments, and required documentation to ensure they can connect successfully with the referral care provider
- Track, collect, and manage data related to patient referrals, including outcomes and service utilization
- Prepare and submit quarterly reports on referral activity, including the number of patients receiving care through partner agencies, in accordance with Memoranda of Understanding (MOU) requirements with hospital system partners
- Provide culturally competent, compassionate support to diverse patient populations
Qualifications:
- Bachelor’s, Nursing degree, or degree in social services, healthcare administration, or related field preferred
- 5+ years' experience working in healthcare (preferably within a hospital system setting), community services, case management, or data tracking/reporting preferred
- Strong interpersonal and communication skills
- Ability to handle sensitive information with professionalism, dignity, and confidentiality
- Detail-oriented with strong organizational and data management skills
- Proficiency in basic data tracking tools (e.g., Excel or electronic health record systems) preferred
- Bilingual skills are a plus
