Case Manager (Harrisburg)
Provide intensive housing case management and crisis support as part of a collaborative program that combines integrated healthcare with the Housing First approach, serving formerly chronically homeless individuals with co-occurring mental health and substance use conditions.
For this role, “clinical experience” may mean different things to different people. In the context of our Integrated Care programs, we are looking beyond traditional case management experience that may primarily focus on referrals, resource navigation, or meeting tangible/basic needs. We value experience working more deeply with individuals experiencing mental illness, substance use disorders, trauma, chronic health conditions, and co-occurring needs within clinically informed or healthcare-integrated environments.
Relevant experience may include work within behavioral health clinics, hospitals, treatment programs, residential care settings, integrated healthcare environments, crisis response programs, or interdisciplinary care teams. Experience facilitating therapeutic groups, teaching coping skills, supporting behavioral health interventions, collaborating with medical or behavioral health professionals, and engaging individuals with complex needs is strongly preferred.
Key Responsibilities:
The essential functions include, but are not limited to the following:
- Deliver intensive case management services and respond to crises as needed.
- Collaborate closely with the Team Lead and an interdisciplinary team to maintain and enhance the project’s impact.
- Support individuals who have formerly experienced chronic homelessness and who have dual diagnoses of serious mental illnesses and substance use disorders.
- Use the Transtheoretical Model and Motivational Interviewing techniques to guide client interactions.
- Apply evidence-based practices for individuals with depression, bipolar disorder, anxiety, and/or substance use challenges, where appropriate.
- Facilitate or co-facilitate client support groups as needed.
- Teach and reinforce coping skills, emotional regulation strategies, wellness planning, and recovery-oriented practices in individual and group settings.
- Participate in interdisciplinary collaboration with behavioral health, medical, psychiatric, and community-based providers to support coordinated client care.
- Support trauma-informed and person-centered interventions that promote housing stability, wellness, and long-term recovery.
- Maintain thorough and accurate electronic and paper records for each client, including electronic medical records.
- Ensure compliance with agency standards for documentation and timely reporting.
Qualifications:
- Bachelor’s degree and at least 2 years of case management, behavioral health, healthcare, or social services experience required; advanced degree in social work or counseling preferred.
- Experience working with individuals experiencing mental health challenges, substance use disorders, and/or chronic health conditions preferred.
- Clinical experience may include work in behavioral health clinics, hospitals, treatment programs, integrated healthcare settings, or multidisciplinary care teams.
- Experience facilitating groups, teaching coping skills, and supporting clients through trauma-informed and recovery-oriented approaches preferred.
- Familiarity with evidence-based practices (such as Cognitive Behavioral Therapy and/or Acceptance and Commitment Therapy), Motivational Interviewing, and the Transtheoretical Model preferred.
- Strong organizational and documentation skills, including maintaining accurate client and electronic medical records.
- Ability to work collaboratively with interdisciplinary teams and community partners.
- Strong communication, crisis intervention, and relationship-building skills.
Job Type: Full-time
Pay: $48,000.00 - $53,000.00 per year
