The Home Care Coordinator Discharge Planner specializes in coordinating home care services and resources for patients transitioning from hospital to home, acting as a discharge planning resource without providing hands-on clinical care.
Work Location: JHH
Weekly Working Hours: Days, 40 hours per week
Job Requirements:

Bachelor of Science in Nursing (BSN) required
-
Licensure in clinical discipline (RN)
-
Active CPR
-
Two (2) years experience in clinical nursing required , previous home care or home care discharge planning experience preferred

Knowledge of Medicare Conditions of Participation preferred

Thorough knowledge of home care community resources, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services preferred.
