General Statement of Duties
The Outreach Coordinator works with MUTEH and the CoC to conduct street outreach, community agency outreach, intakes and program placement. The Outreach Coordinator works to locate potential client and connect them to an appropriate housing program.
Duties includes working with the case management team and the housing team directly to engage and conduct intakes with eligible applicants. Services provided includes conducting outreach to applicants and providers, making appropriate referral, adding applicants to the Coordinated Entry System (CES), connecting appropriate applicants from CES to appropriate housing programs, conducting paper intakes, and collecting required documentation of program participants. The principal duties are performed both in a general office environment and in the field and community where program operates.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- Reports directly to the Program Director and Team Lead.
- Coordinate and lead outreach activities in MUTEH’s programs coverage area.
- Ability to work with other MUTEH team members to reach monthly goals for participants’ enrollment into the program by engaging applicants, referring them to appropriate programs, completing initial paperwork/documentation.
- Prepare and maintain required charting and documentation in a timely and thorough manner (including HMIS input).
- Complete comprehensive needs assessment with program applicant before program enrollment including developing initial goals for housing stability.
- Conduct screenings with clients to determine program eligibility.
- Coordinate with case management team to ensure all participants who are engaged and referred to services are connected to case management and housing.
- Collaborate and seek to develop new relationships with representatives in the community and other agencies to increase engagement with homeless individuals.
- Act as a liaison between client and community agencies as needed.
- Assist participants by providing linkage to other services and divert applicants when appropriate; maintaining successful linkages.
- When needed, transport participants in vehicle in a safe, cautious, and responsible manner.
- Document all engagements with applicants in HMIS Street Outreach module.
- Collect appropriate eligibility documentation on program applicants.
- Maintain contact with Program Director and Team Lead regarding referrals to program.
- Participate in weekly supervisory and/or staff meetings, training and development opportunities as determined appropriate by the Program Director.
- Other duties as assigned.
QUALIFICATIONS AND KEY COMPETENCIES
- An Associate’s Degree, or a minimum of two (2) years case management experience is preferred. Experience working with homeless population(s) is helpful.
- As the outreach work spans a number of counties in MUTEH’s coverage area, this position requires regular travel in personal and/or agency vehicles. Outreach Coordinators must have a valid driver’s license, their own personal vehicle and proof of personal vehicle insurance.
- Ability to work with diverse populations and be able to meet clients where they are.

- Strong organizational skills, attention to detail, and ability to meet deadlines consistently.
