We are seeking a compassionate and skilled Speech Language Pathologist (SLP) PRN to join our home health care team. The Home Health SLP will provide specialized speech, language, and swallowing therapy to patients in their homes, helping individuals regain communication skills and improve quality of life. This role requires excellent clinical expertise, strong interpersonal skills, and the ability to work independently in a home health setting.
Field-based role requiring travel to patients’ homes within the Kansas City, MO area.
Flexible scheduling to accommodate patient needs. Looking to fill 10 to 15 hours a week.
Exposure to diverse home environments; must adhere to safety and infection control guidelines
Key Responsibilities:
- Conduct comprehensive assessments to evaluate patients’ speech, language, cognition, voice, fluency, and swallowing abilities.
- Develop individualized treatment plans based on patient needs, medical history, and functional goals.
- Provide therapy to address conditions such as speech disorders, language impairments, cognitive-communication deficits, and dysphagia.
- Educate patients and their families on communication strategies, exercises, and techniques to support therapy goals.
- Monitor and document patient progress, adjusting treatment plans as needed to optimize outcomes.
- Collaborate with physicians, nurses, physical/occupational/respiratory therapists, and other healthcare professionals to ensure coordinated care.
- Maintain accurate and timely clinical records in compliance with regulatory standards and agency policies.
- Provide guidance on adaptive equipment, augmentative communication devices, or dietary modifications as needed.
- Ensure patient safety and comfort during home visits, adhering to infection control and safety protocols.

Qualifications
- Master’s degree in Speech-Language Pathology from an accredited program.
- Current and valid Speech-Language Pathologist license in Kansas and Missouri.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA). Clinical Fellowship Year (CFY) under consideration.
- Minimum of [1-2 years] of experience in speech-language pathology; home health experience preferred.
- Strong knowledge of speech, language, and swallowing disorders across adult and geriatric populations.
- Excellent communication and interpersonal skills to build rapport with patients and families.
- Ability to work independently, manage time effectively, and travel to patients’ homes.
- Valid driver’s license, reliable transportation, and auto insurance.
- Proficiency in electronic medical record (EMR) systems and documentation.
- CPR certification.
