Please Note: This position requires prior experience in an administrator role within home care or home health and a minimum of three (3) years of Client Care Coordination experience. Applicants who do not meet the required qualifications will not be considered. An active CNA or LPN license is preferred.
Position Overview
This part-time role blends office and field work — client/prospect visits, community outreach, and quality assurance — and requires strong computer skills plus the ability to quickly pick up new software. You'll manage multiple priorities, help ensure client satisfaction, and adapt services as client needs evolve.
Key Responsibilities
Client Intake & Service Planning
- Respond to inquiries, referrals, and intake calls; convert leads into initial consultations
- Complete intake assessments and set up new client services
- Develop and update service plans based on client needs and goals
Scheduling & Caregiver Supervision
- Schedule caregivers based on client needs, availability, and qualifications; cover open shifts
- Introduce caregivers to new clients and ensure proper training
- Conduct supervisory visits, performance evaluations, and coaching
Client Relationship Management
- Conduct in-home consultations, QA visits, and follow-ups
- Resolve client concerns promptly and maintain ongoing communication
- Report changes in client condition to the RN Manager
- Community outreach including hosting events
On-Call & Documentation
- Take after-hours and weekend emergency calls and manage related scheduling
- Complete visit notes and service records accurately and on time
- Maintain confidentiality and compliance with agency policies
- Ensure compliance with DCH regulations and standards through ongoing reviews and timely updates to client and caregiver records and documentation

Minimum Requirements
- Active CNA or LPN license (preferred)
- Minimum of three (3) years of Client Care Coordination experience (required)
- Prior experience in an administrator role within home care or home health (required)
- Valid driver's license, proof of auto insurance, and reliable transportation
- Strong communication and problem-solving skills
- Proficient computer skills; able to learn new platforms quickly
- Available to work Monday–Friday
- Experience with call intake, scheduling, and client service
- Ability to take emergency calls related to scheduling as needed after hours
Pay: $24.37 - $29.35 per hour
