Job Overview:
We are seeking a dedicated and proactive Community Healthcare Coordinator to serve as a vital link between healthcare providers, community organizations, and residents. The ideal candidate will possess a strong passion for community health, excellent communication skills, and the ability to coordinate resources effectively. As a Community Healthcare Coordinator, you will be responsible for developing outreach initiatives, facilitating access to healthcare services, and promoting health education within diverse populations. This role offers an opportunity to make a meaningful impact on community well-being through organized efforts and compassionate engagement.
Duties
- Plan and implement community outreach programs aimed at increasing awareness of available healthcare services and resources.
- Collaborate with local clinics, hospitals, and social service agencies to coordinate care delivery and support services.
- Conduct needs assessments within the community to identify gaps in healthcare access and develop targeted interventions.
- Educate residents on health topics such as preventive care, chronic disease management, and wellness practices through workshops and informational sessions.
- Serve as a liaison between community members and healthcare providers to facilitate appointments, follow-ups, and resource navigation.
- Maintain accurate records of outreach activities, participant engagement, and program outcomes for reporting purposes.
- Foster relationships with community leaders and organizations to strengthen partnerships and promote health equity.
- Stay informed about public health policies, funding opportunities, and best practices relevant to community health initiatives.
Qualifications
- Bachelor's degree in Public Health, Social Work, Nursing, or a related field; relevant experience may substitute for formal education.
- Proven experience in community outreach, health education, or case management within diverse populations.
- Excellent interpersonal skills with the ability to engage effectively with individuals from various cultural backgrounds.
- Strong organizational skills with the capacity to manage multiple projects simultaneously.
- Knowledge of local healthcare systems, social services, and public health resources.
- Ability to work independently as well as collaboratively within multidisciplinary teams.
