The Role
We are seeking a highly organized, empathetic Care Coordinator to join our team. Please note that this is a dynamic, hands-on position; you must be willing to drive to multiple locations and will be assigned to various client homes throughout the community, as well as to our main office. Additionally, Care Coordinators must be willing to step in and perform Direct Support Professional (DSP) duties and other administrative duties as needed to ensure continuous, safe client care.
In this role, you will collaborate with case management teams and serve as a key liaison among clients, families, and DSPs. You will support medical case management and help oversee daily ISLA service delivery, ensuring each person on your caseload receives comprehensive, person-centered care that aligns with their goals, health needs, and state regulations.
Key Responsibilities
- Medical & Case Management: Collaborate directly with case management teams to oversee client health. Arrange and personally attend doctor appointments, manage all aspects of client medications (monitoring usage, ordering refills), and guarantee that care plans, habilitation plans, and service plans are actively and accurately implemented in each home.
- Care Planning: Develop, implement, and monitor individualized care plans and ISLA goals in collaboration with clients, families, and Desert Regional Center (DRC) Service Coordinators.
- Care Coordination: Coordinate with healthcare providers, insurance companies, and patients to ensure smooth flow of information and timely care delivery.
- Team Leadership: Supervise, train, and support a team of Direct Support Professionals (DSPs). Coordinate staff schedules to ensure consistent 24/7 client coverage.
- Direct Support Coverage: Perform Direct Support Professional (DSP) duties and provide direct, hands-on client care as needed to cover shifts, manage emergencies, or address staffing shortages.
- Quality Assurance & Travel: Conduct regular home visits across multiple assigned locations to ensure living environments are safe, sanitary, and compliant with all state and agency regulations.
- Documentation & Compliance: Audit daily logs, medication administration records (MARs), and incident reports. Ensure all documentation meets Nevada Medicaid and Regional Center standards.
- Advocacy & Communication: Act as a point of contact for families, medical professionals, and direct support professionals to advocate for the client's needs.
- Crisis Management: Provide on-call support and guidance to staff during emergencies or behavioral escalations.
Qualifications
- Experience: Experience in human services, particularly supporting individuals with intellectual or developmental disabilities, is preferred but not required. Previous experience in an ISLA program is a plus.
- Skills & Flexibility: Exceptional organizational, communication, and problem-solving skills. Ability to manage schedules, track detailed medical needs, handle high-stress situations, and adapt to stepping into direct care (DSP) roles when required.
- Education: High School Diploma or equivalent (Bachelor’s degree in Social Work, Psychology, Nursing, or related field is a plus).
- Mobility & Travel Requirements: Valid Nevada Driver’s License, reliable transportation, and valid auto insurance are essential. You must be willing to drive to multiple locations, as you will be assigned to various client homes, medical facilities, and the main office to conduct visits, attend appointments, and complete administrative tasks.
- Background: Must pass a state and federal background check.
- Drug Testing: Must be able to pass Pre-employment Drug Testing.
- Certifications: Current CPR/First Aid certification (or willingness to obtain upon hire).

Pay: $18.00 - $20.00 per hour
