Registered Nurse (RN) Hospice Case Manager
Oakstone Hospice is seeking a compassionate and skilled RN Hospice Case Manager to join our interdisciplinary team. This role provides direct patient care, care coordination, and support to patients and families throughout the end-of-life journey.
Responsibilities
- Manage a caseload of hospice patients in home and facility settings
- Complete initial and ongoing nursing assessments
- Develop and update individualized plans of care
- Provide symptom and pain management focused on comfort and quality of life
- Coordinate care with physicians, social workers, chaplains, CNAs, and other team members
- Educate patients and families on medications, disease progression, and end-of-life expectations
- Respond to changes in patient condition and provide timely interventions
- Complete accurate, timely documentation per hospice and Medicare regulations
- Participate in interdisciplinary team meetings
Qualifications
- Active RN license in the state of practice
- Minimum 1–2 years of nursing experience
- Hospice experience preferred minimum of 1 year (training available for the right candidate)
- Strong assessment, communication, and time-management skills
- Ability to work independently in the field
- Valid driver’s license and reliable transportation

Pay: $80,000.00 - $90,000.00 per hour
