Job Overview
We are seeking a compassionate and dedicated Hospice Home Health Aide to provide exceptional in-home care to patients nearing the end of life. This vital role involves supporting patients with their daily needs, ensuring comfort, dignity, and quality of life. As a Hospice Home Health Aide, you will work closely with families, medical teams, and community resources to deliver personalized care that aligns with home healthcare regulations and best practices. Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
- Monitor and document patient condition by observing vital signs, recording medical information in Electronic Health Record (EHR) systems, and reporting changes to healthcare professionals promptly.
- Provide medication reminders and assist with medication administration under supervision, ensuring adherence to medical terminology and safety protocols.
- Support patients experiencing dementia or Alzheimer's disease through specialized memory care techniques, ensuring safety and comfort.
- Maintain patient confidentiality by adhering to HIPAA regulations while handling medical records and sensitive information.
- Assist with end-of-life care by offering emotional support, managing symptoms, and ensuring patient comfort in accordance with hospice care standards.
- Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe environment for patients at all times.
Requirements
- Proven experience working as a Home Health Aide or in assisted living, nursing home, or long-term care settings.
- Demonstrated ability in patient observation, taking vital signs, catheter care, and assisting with ADLs.
- Knowledge of home health care assistance regulations and familiarity with EMR/EHR systems for accurate medical documentation.
- Certifications in First Aid and Basic Life Support (BLS), including CPR training.
- Experience working with individuals with disabilities or developmental disabilities preferred; experience in dementia or Alzheimer's care is highly valued.
- Ability to lift heavy objects safely and perform physical tasks such as patient transfer methods using equipment like Hoyer lifts or patient transfer techniques.
- Excellent communication skills for effective client interaction and collaboration with healthcare teams.
- Commitment to maintaining patient confidentiality and adhering to HIPAA standards.
- Flexibility to work live-in shifts or varied hours as needed; previous home health agency experience is a plus.
