Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to inform tailored care plans.
- Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards.
- Manage complex wound care, including dressing changes and aseptic techniques for sterile procedures.
- Provide specialized care for patients with conditions such as Alzheimer’s disease, dementia, developmental disabilities, or those requiring hospice or palliative support.
- Educate patients and family members on disease management, medication administration, nutrition, and lifestyle modifications to foster independence.
- Utilize electronic health record (EHR) systems like Epic or Cerner to document patient interactions accurately and efficiently.
- Coordinate with multidisciplinary teams for discharge planning, case management, and referrals to community resources or specialty services such as cardiology or pulmonology.
Experience
- Valid RN or LPN license in good standing with the state licensing board.
- Prior experience in home health care is highly preferred; experience in acute care settings such as ICU, emergency room, or hospital medicine is a strong plus.
- Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth.
- Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards aligned with HIPAA regulations.
- Hands-on experience with IV insertion, catheterization, venipuncture, phlebotomy, and sterile techniques required for safe patient care.
- Background working with diverse populations including pediatrics (neonatal ventilator management), geriatrics (memory care), behavioral health patients, and individuals with disabilities.
- Critical thinking skills demonstrated through triage responsibilities and urgent care scenarios; ability to perform physical assessments and diagnostic evaluations confidently.
- Strong understanding of physiology principles related to respiratory management (ventilators), cardiac care (cardiac catheterization), infection control practices, and pain management strategies.
