Open Water Counseling & Recovery is growing, and we are looking for a passionate therapist to join our team serving nursing homes and assisted living communities.
At Open Water, we believe mental health care should reach people where they live. Our therapists work directly within long-term care communities, providing compassionate counseling and becoming a trusted part of the care team.
This is a field-based position, allowing you to build meaningful relationships with residents while collaborating closely with nursing staff, physicians, and interdisciplinary teams. Our work focuses on improving quality of life for older adults by addressing depression, anxiety, grief, adjustment to illness, and other mental health challenges that often accompany aging.

Responsibilities
- Provide individual counseling to residents in nursing homes and assisted living communities
- Conduct mental health assessments and diagnostic evaluations
- Develop individualized treatment plans
- Implement evidence-based therapeutic interventions
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with interdisciplinary care teams
- Maintain accurate and timely documentation
Qualifications
- Master’s degree in Social Work, Counseling, or related mental health field
- Independently Licensed in Ohio (LISW or LPCC)
- Experience working with older adults preferred
- Strong communication and relationship-building skills
- Ability to work independently in the field while collaborating with a clinical team
- Passion for supporting the mental health and well-being of the geriatric population

Pay: From $70,000.00 per year
