Job Summary
We are looking for a Occupational Therapist who is willing to work per-case, part time in a home health setting. You will evaluate and treat patients with the goal of improving their condition and restoring their functionality.
Key Responsibilities and Duties
- Conduct initial and ongoing comprehensive assessments, including evaluations of activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, cognition, and home safety.
- Develop and implement individualized treatment plans based on physician orders and clinical best practices.
- Administer occupational therapy interventions to improve strength, coordination, range of motion, fine motor skills, cognitive functioning, and independence with daily activities.
- Assess the home environment and recommend adaptive equipment, assistive devices, and environmental modifications to improve patient safety and functional independence.
- Educate patients, families, and caregivers on therapeutic exercises, energy conservation techniques, fall prevention, adaptive equipment use, and safe performance of daily activities.
- Monitor patient progress, modify treatment plans as appropriate, and document clinical findings, patient responses, and outcomes in a timely and organized manner.
- Supervise Occupational Therapy Assistants (OTAs) and Home Health Aides when involved in the patient's plan of care.
- Collaborate with physicians, nurses, physical therapists, speech-language pathologists, social workers, and caregivers to coordinate comprehensive patient care.
- Conduct discharge planning and complete occupational therapy discharge summaries when patients have achieved treatment goals or no longer require skilled occupational therapy services.
- Ensure compliance with documentation standards, professional regulations, and agency policies.
- Advocate for patient needs and assist with connecting patients to appropriate community resources and support services.
- Maintain professional competency through continuing education, annual in-service training, and participation in agency quality improvement initiatives.
Education & Licensing
- Must be licensed Occupational Therapist in State of Arizona.
Certifications
- Valid BLS certification (e.g., American Red Cross, AHA).
