We are seeking a passionate and energetic Case Manager to join our Youth Homeless Services team. In this vital role, you will directly support youth experiencing homelessness by developing individualized care plans, connecting them with essential resources, and fostering their path toward stability and independence. Your proactive approach will help empower youth to overcome barriers and build brighter futures through comprehensive case management, crisis intervention, and motivational interviewing techniques.
Purpose:
The Case Manager for our Transitional Living Program is tasked to create an environment of trust and compassion that allows young adults to grow and develop. To fully assess their diverse needs, to provide psycho-social support, to link current or former young adults with much needed resources and services, conduct community outreach, and to conduct aftercare services for our Transitional Living Program. The TLP Case Manager will ensure that all individual plans and all services are provided in accordance with the Youth Emerging Stronger mission statement, Value Statement, Code of Conduct and YES Policies and Procedures. This position requires an individual who can actively contribute to a positive, stable, responsive and responsible environment for change.
Responsibilities and Duties:
· Cultivate an environment of caring that offers our young adults a chance to develop positive relationships and connections with adults.
· Maintain an attitude of professionalism, collegiality, and optimism with both staff and youth to promote the welfare and well-being of the youth.
· Provide strengths based case management and service coordination services designed to assist young adults in obtaining permanency.
· Oversee and/or conduct comprehensive intakes and psychosocial assessments of each young adult.
· Use all available data to create an individualized service plan that identifies youth's strengths and weaknesses; help young adults to plan short-term and long-term goals.
· Cultivate a working knowledge and relationship with relevant community resources to create linkages that meet the needs of the young adults.
· Maintain accurate and timely documentation that meets legal requirements; complete monthly and weekly reports.
· Periodically transport and accompany young adults to meetings and appointments with medical providers, court as directed.
· Assist in the training and guidance of interns on relevant agency policies and procedures.
· Conduct Aftercare Services for former youth
· Conduct Outreach services when needed
· Attend all staff meetings, in-services, supervisions and conferences as directed.
· Perform other related duties as assigned
Minimum Qualifications:
· Bachelor’s in Social Work or related field Required
· One – two years of related experience working with diverse, vulnerable populations.
· Ability to effectively manage behavioral and emotional problems presented by young adults
· Ability to multi-task and prioritize and respond to multiple needs
· Great communication Skills negotiation
· Ability to pass a criminal background check
· Reliable attendance and punctuality
· Valid CA Driver’s License
Physical Demands:
- Sitting at desk with intermittent walking, standing, bending, and occasionally lifting light objects.
Pay: $25.00 - $27.00 per hour
Expected hours: 40.0 per week
