Serving patients from 3 clinic locations, and 8 affiliated hospital & surgery center affiliations.

Position
- Doctor of Optometry (OD) from an accredited optometry school.
- Current unrestricted or eligible Wisconsin State license
- Active or eligible Medicare/Medicaid Enrollment
- After hours call with ophthalmology support is required
