Lam Clinic is a growing functional medicine practice dedicated to providing exceptional, patient-centered care. We are looking for a compassionate, organized, and detail-oriented Chronic Care Manager to join our team. This position plays an important role in supporting our members by providing excellent customer service and ensuring they receive the highest level of care.
Responsibilities
As a Chronic Care Manager, you will serve as a primary point of contact for our members while providing administrative support to our clinical team. Responsibilities include, but are not limited to:
- Answering incoming membership calls and returning member phone calls
- Responding to text messages and member inquiries
- Scheduling appointments
- Processing medication refill and referral requests
- Reviewing medical records and laboratory results for provider follow-up
- Onboarding new members and ensuring a smooth enrollment experience
- Sending appointment and meeting reminders
- Maintaining membership accounts and records
- Supporting member retention through exceptional customer service and follow-up
- Gathering and documenting member feedback & exit interviews
- Performing additional administrative duties as assigned
Qualifications
- Previous experience in a medical office, healthcare, or customer service environment preferred
- Strong communication and interpersonal skills
- Excellent organizational and multitasking abilities
- Ability to prioritize work in a fast-paced environment
- Professional phone etiquette
- Experience with electronic medical records (EMR/EHR) is a plus
- Proficient with Microsoft Office and general computer applications
- High attention to detail and accuracy
- Positive attitude and team-oriented mindset
- 2-3 years of Patient Care Coordination and Member/Patient Communications
