Job Overview
Evolve Health and Wellness is seeking a highly skilled and motivated Nurse Practitioner or Physician Assistant to join our growing health, wellness, and longevity medicine team. This role is ideal for a provider with experience or strong interest in hormone optimization, medical weight loss, peptide therapy, IV therapy, regenerative medicine, and preventative health.
The ideal candidate is clinically confident, patient-focused, and passionate about helping patients look, feel, and perform their best through evidence-informed, personalized care. Responsibilities
Evaluate patients and develop customized treatment plans based on symptoms, goals, medical history, and lab work.
Manage hormone optimization programs, including TRT, BHRT, and related therapies when appropriate.
Support medical weight loss programs, including GLP-1 therapies and lifestyle guidance.
Educate patients on peptide therapy, longevity medicine, IV therapy, vitamin injections, and regenerative health options.
Order, review, and interpret labs as part of patient care planning.
Perform or oversee treatments such as injections, IV infusions, and wellness-based therapies.
Maintain accurate and detailed documentation in EMR/EHR systems.
Provide excellent patient education, follow-up, and ongoing care management.
Collaborate with the clinical and leadership team to improve protocols, patient experience, and service offerings.
Help contribute to the growth of the practice through professionalism, patient retention, and clinical excellence.
Qualifications
Active Nurse Practitioner or Physician Assistant license.
Experience in hormone optimization, medical weight loss, functional medicine, regenerative medicine, or longevity medicine preferred.
Knowledge of peptides, GLP-1 therapies, TRT/BHRT, IV therapy, and preventative health is strongly preferred.
Comfortable interpreting labs and creating individualized care plans.
Strong communication skills and ability to educate patients clearly and confidently.
Passion for health optimization, longevity, and patient-centered care.
Experience in a cash-pay, concierge, aesthetics, wellness, or medical office setting is a plus.
Must be professional, reliable, team-oriented, and committed to delivering an exceptional patient experience.
Compensation
Compensation is competitive and based on experience, skill set, availability, and role structure. This position may be hourly, salaried equivalent, or a combination structure, with compensation ranging from approximately $30–$60 per hour, plus potential commission or performance-based incentives.
