Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) specialized in Memory Care to join our dynamic healthcare team. Responsibilities
- Deliver comprehensive nursing care to residents with memory impairments, including Alzheimer’s disease and other dementias, ensuring their safety and comfort at all times
- Conduct detailed patient assessments, monitor vital signs, and document medical information accurately using electronic health record (EHR) systems such as Epic or Cerner
- Administer medications, including injections, IV infusions, and pain management therapies, following strict protocols and HIPAA regulations
- Assist with activities of daily living (ADLs), including mobility support, catheter care, tube feeding, and hygiene assistance tailored to individual needs
- Collaborate with interdisciplinary teams to develop and implement personalized care plans that address cognitive, physical, and emotional health goals
- Manage acute pain episodes and respond swiftly to behavioral health concerns or emergencies within the memory care unit
- Maintain sterile techniques during procedures such as phlebotomy, IV insertion, suturing, or specimen collection while adhering to infection control standards
Skills
- Proven experience working with individuals with developmental disabilities, dementia care, or in memory care settings
- Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques relevant to senior care
- Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation and patient records management
- Skilled in medication administration including injections and IV therapy
- Ability to perform physical examinations, vital sign assessments, and patient assessments efficiently in a fast-paced environment
- Knowledge of infection control practices and aseptic techniques essential for safe patient care
- Excellent communication skills for effective triage, family education, and team collaboration; comfortable working with seniors experiencing cognitive decline or behavioral challenges
