Under the direction of the Case Management Coordinator, the Case Manager/Intake Worker will be responsible for going into the home to assess or reassess elderly clients, reporting to the Case Management Coordinator, and following up on each client as needs dictate. The Case Manager/Intake Worker will provide counseling, information and/or assistance related to Medicare or other health insurance for the Virginia Insurance Counseling and Assistance Program (VICAP) and SMP (Senior Medicare Patrol) Program.
To visit clients (Title III, private pay, Adult Day Care Residences, clients preparing to enter an ACR), in the homes of those assigned through initial contact forms, annual reassessments of clients or bi-annual reassessments of case management clients, filling out all required paperwork and documenting in the files contact with the client, including home visits or telephone contact. To prepare records and required case information for presentation at the Case Management Team meetings and to do any necessary follow up. To attend meetings, educational functions and training as delegated. To prepare client records, and any reports deemed necessary on or before deadline. Administers the Virginia Insurance Counseling and Assistance Program (VICAP) and SMP by educating, counseling and assisting older adults and their caregivers on disability, medical benefits programs, long term care insurance, and problems with Medicare, Medicaid, medical bill paying and fraud. Attends required VICAP training, prepares displays, provides presentations, publicizes the VICAP program, oversees data reporting, distributes informational materials, provides administrative support, and recruits volunteers, complete reports on a monthly basis and submits as appropriate to the state in a timely manner. To maintain client confidentiality by both placing any checked out files back in the file room in a timely manner and in not discussing the client’s file with anyone not directly related to services for that client. To perform other duties deemed necessary that will enhance the program and LCAAA as assigned. This is a full-time position and the nature of it calls for the employee to have the ability to work with an elderly population in a sensitive and confidential manner. The position requires excellent verbal and written communication skills, a high degree of organization, and the ability to coordinate services. This person needs to be able to work independently when performing position duties under general supervision, yet be a team member of the department and the agency. This person needs to have good communication and interviewing skills as well as knowing the principles of file documentation and service planning.
In addition, the case manager/intake worker needs skills in identifying and documenting a client’s needs, have knowledge of community as well as agency resources to meet these needs, and be able to coordinate services for clients. The case management intake assistant should also be able to assess clients using the Virginia Uniform Assessment Instrument and be able to accurately develop a Plan of Care for the client. He/she should have knowledge of the aging process and understand the impact of illness/disability on the elderly, knowledge of consumer’s rights, and available community resources.
Minimum of High School Diploma; 1 year Associates or higher, in Human Services or Applied Sciences preferred. Ability to work with the elderly, good skills in both written and verbal communication, ability to maintain records and work independently with little supervision. Must possess experience in the areas of public relations. Must maintain a driving record of "0" points or better andbe insurable under the agency's Certificate of Insurance for vehicle coverage
