Full-time position that will provide support and medical social services to terminally ill patients and their families. Also, the social worker is responsible for the development, implementation, and management of psychosocial services provided to hospice patients and their families in the home care or residential setting. Excellent verbal and communications skills. Master’s Degree required and License in Social Work preferred. Minimum of 3 years experience in hospice/home care, acute care setting or long-term care background preferred. Flexible schedule, weekend rotation, ability to travel throughout the Texas area. Bilingual preferred.
Must have a Masters in Social Work and be LCSW
Responsibilities
- Conduct comprehensive patient assessments to determine needs and develop individualized care plans.
- Provide clinical counseling and behavioral therapy to clients, utilizing techniques such as cognitive behavioral therapy and motivational interviewing.
- Maintain accurate medical documentation and adhere to HIPAA regulations.
- Collaborate with multidisciplinary teams to ensure holistic patient care in home and community settings, including hospice care.
- Engage in crisis intervention and support families through challenges, particularly in child and family counseling contexts.
Pay: From $66,000.00 per year
