Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine health status.
- Administer medications, injections, IV infusions, and other treatments following strict protocols and HIPAA regulations to ensure safety and efficacy.
- Manage complex clinical procedures such as catheterization, tube feeding, wound suturing, and ventilator management in various healthcare settings.
- Utilize electronic health record (EHR) systems like Kinnser , Wellsky for accurate medical documentation and case management.
- Conduct triage assessments to prioritize patient needs in urgent or emergency situations across emergency rooms, urgent care clinics, or hospital environments.
- Collaborate with multidisciplinary teams to develop discharge plans, coordinate post-acute care, and provide health coaching tailored to individual needs.
- Support infection control practices and sterile techniques to maintain a safe environment for patients and staff alike.
Qualifications
- Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).
- Proven experience in Home Health
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for medical documentation.
- Experience working with diverse populations including individuals with developmental disabilities, dementia (Alzheimer’s care), memory care needs, pediatric patients, neonatal ventilator management, or hospice care is advantageous.
- Strong understanding of anatomy, physiology, pharmacology, and medical terminology to perform effective patient assessments and interventions.
- Ability to handle high-pressure situations with professionalism during acute pain management or emergency medicine scenarios.
