Most patients with COPD or heart disease never make it to rehabilitation. Not because they don't need it, but because getting there is too hard. Bloom Medical Group exists to change that. We bring pulmonary and cardiac rehabilitation directly into patients' homes through telehealth, removing every barrier between a chronic disease diagnosis and the program that can genuinely improve someone's quality of life.
We are growing fast across California and multiple other states, and we are building a clinical team that believes access to rehabilitation care should not depend on where you live or whether you can drive.
The Role
You are the patient's first clinical point of contact and the clinician who makes their program possible. Before your encounter, the patient completes a structured intake covering their medical history, symptom scores, and insurance information. You review that intake, conduct a live telehealth encounter, assess the patient under physician-delegated authority per California Business and Professions Code Section 3516, confirm medical necessity, and establish the patient-provider relationship.
From there you sign the individualized treatment plan, conduct 30-day reassessments as the patient progresses, and manage any clinical escalations that arise. Your clinical judgment is what opens the door for every patient you see.
Requirements
Every candidate must have all of the following to be considered.
- Active, unrestricted Physician Assistant license in California
- Currently in-network and credentialed with at least two of the following: Cigna, Aetna, Anthem BCBS, or UnitedHealthcare
- Active CAQH profile — you must include your CAQH ID in your application
- Minimum 2 years of telehealth clinical experience
- NCCPA PA-C certification
- BLS certification
- Must be willing to operate under a supervising physician arrangement per California Business and Professions Code Section 3516
Strongly Preferred
- Experience in pulmonary, respiratory, cardiac, primary care, or chronic disease management
- Licensed in multiple states
- PA Compact privilege holder
- Existing credentialing with all four payers listed above
