Job Summary
We are seeking a compassionate and dedicated Farsi/English or Mandarin Chinese/English bilingual Social Worker/ Social Worker Assistant to join our journey (Costa Mesa Adult Day Health Care Center). The ideal candidate will be responsible for providing holistic support to seniors and families facing various challenges. This role involves conducting assessments, developing treatment plans, and implementing interventions to improve the well-being of seniors. The Social Worker will work collaboratively with other professionals such as RN, Activity leader and Program Director.
Required Qualifications
- Legal to work in the U.S.
- One of the following degrees: Master's degree in Social Work, Psychology, Gerontology, Counseling (Social Science) OR Bachelor's degree in Social Work
- Farsi (Fluent), Mandarin Chinese (Fluent), English (Fluent)
- Knowledge of Microsoft Office, Word & Excel

Responsibilities
- Conduct patient assessments to determine needs and develop individualized care plans.
- Provide behavioral therapy and clinical counseling to clients, utilizing techniques such as cognitive behavioral therapy and motivational interviewing.
- Maintain accurate documentation and records in compliance with HIPAA regulations.
- Collaborate with families and community resources to support clients.
- Cooperate with other department member such as Program Director, Registered Nurse and Activity Coordinator.
