Home Health Care, Inc. is looking for an experienced Occupational Therapist to specialize in Start of Care and admission visits throughout the Twin Cities Metro. Rather than spending your week carrying a traditional treatment caseload, you'll play a critical role at the very beginning of a patient's home health journey.
Be the Clinician Who Gets Everything Started Right
The first home health visit matters.
You're often meeting a patient shortly after a hospitalization, surgery, illness, or significant change in function. They may be overwhelmed. Their family may have questions. The home may present challenges no one identified before discharge.
Your job is to walk in and make sense of it.
You'll assess the patient, evaluate the home, identify immediate concerns, establish goals, complete the necessary admission documentation, and help determine what services will give that patient the best opportunity to remain safely at home.

Your primary opportunity is during the week, but we know some clinicians appreciate having the ability to earn more when it works for their schedule.
That's why weekend Start of Care visits are optional and paid at the increased rate of $120 per visit.
There's no required weekend rotation.
When you want the additional opportunity, you can make yourself available.
This Is a Role for an OT Who Likes to Think
Start of Care isn't about following the same routine from one patient to the next.
One patient may be returning home after a joint replacement.
The next may have experienced a stroke.
Another may be struggling with progressive weakness, cognitive changes, falls, or a complex medical condition.
You need to be able to assess the situation, identify priorities, communicate effectively, and establish an appropriate plan of care.
That variety is exactly what makes this position appealing to experienced clinicians.
What You'll Do
- Complete comprehensive Start of Care and admission evaluations in patient homes.
- Assess ADLs, functional mobility, cognition, upper-extremity function, home safety, and caregiver support.
- Complete OASIS and admission documentation accurately and within required timeframes.
- Develop individualized plans of care based on patient goals and clinical findings.
- Identify fall risks and barriers to safe independence.
- Recommend adaptive equipment, home modifications, and compensatory strategies.
- Provide initial patient and caregiver education.
- Coordinate with physicians, nurses, PTs, SLPs, social workers, and other clinicians.
- Communicate findings and establish a strong clinical foundation for the clinicians who will follow the patient.
- Complete accurate and timely electronic documentation.
We're Looking for Clinical Confidence
This position is especially well suited for an Occupational Therapist who enjoys being trusted to make decisions.
You'll have the support of an experienced clinical team, but you'll also spend much of your day independently evaluating patients and determining what they need.
Required Qualifications
- Active Minnesota Occupational Therapist (OTR/L) license
- Current CPR certification
- Valid driver's license and reliable transportation
- Strong assessment and clinical decision-making skills
- Excellent documentation and communication abilities
- Willingness to travel throughout an approximately 75-mile radius based on referral needs
Highly Preferred
- Home health experience
- Start of Care / admission experience
- OASIS experience
- Knowledge of Medicare home health requirements
- Experience with medically complex and geriatric patients
