NATURE OF WORK: To coordinate the development, implementation, and oversight of the LEAD program for adult men and women with a history of low-level criminal behavior related to drug abuse and individuals arrested for low level drug offenses. These duties include, but are not limited to: planning, developing, coordinating, and assisting in the implementation of all treatment and education activities of the LEAD program to meet the client’s needs in order to facilitate their recovery and rehabilitation.

Identify and divert low-level drug offenders into immediate treatment and support services as a diversion tactic from arrest and incarceration.
- LEAD participants are linked to trauma-informed care and /or co-occurring behavioral health services.

Form partnerships and active relationships with local, county, and state law enforcement departments beneficial to produce referrals between law enforcement and behavioral health facilities.

Reduce reoccurring arrest rates (recidivism) among the client’s that are referred through diversion and treatment services.

Increase the number of individuals actively engaged in specialized treatment and remove treatment gaps.

Work actively with a minimum of twenty (20) clients.

Provide mentoring through life experience to individuals experience substance abuse problems.

Meet face-to-face with the individual and identified supports and/or by secured video conference at a frequency and format requested and agreed upon by the individual. The meetings can occur in a facility or the community.

Develop Recovery Wellness Plans with clients and monitor those plans with them in scheduled or unscheduled meetings as identified above.

Link and document clients to available resources including but not limited to continuing education, professional development, safe housing, recovery support groups/systems, self-help groups, medical and health facilities, and substance abuse and/or mental health services.

Help individuals discover, access, and utilize ways top remain alcohol and drug free and/or reduce harm associated with their substance use behaviors.

Help individuals find best practice resources from harm reduction, detoxification, treatment, family/community/peer support and education, and local or online support groups, as well as help individuals create a change plan for their recovery.

Provide 24-hour service to assist law enforcement in connecting individuals to the treatment services.

Provide transportation to appointments when needed to assist clients in being successful with therapy and recovery support services.

Collaborate with the Quick Response Team and State Law Enforcement on the Angel Initiative to provide clients with a warm hand-off to treatment transaction.

Provide data as requested by the evaluation team.

Document all activated information related to service area via a cloud-based care coordination software platform.

Monitor documentation to ensure compliance with Valley Healthcare guidelines.

Adheres to professional code of ethics and Valley Healthcare’s Standards-of-Conduct in performing and overseeing services.

Maintain all agency and/or professional training requirements (Mandt, First-Aid/CPR, CEU and all other training as required by Valley Healthcare).

Continue to develop professionally by maintaining professional and technical knowledge by attending educational workshops, adhering to professional standards, reviewing professional publications, and establishing personal work related networks.

Serves on agency, state, and community committees as well as local boards and task forces as appropriate.

Other work-related duties as assigned.
MINIMUM QUALIFICATIONS:

High School diploma or certified high school equivalency.
-
Knowledge of treatment resources, including a working knowledge of the 12-Step program and fellowships.

Prior experience working with law enforcement, court systems, and diversion programs.

Knowledge of the medical model of addiction treatment, including but not limited to: symptoms, stages of the disease, process of recovery, and relapse prevention.

Knowledge of the various therapeutic techniques in addressing the chemical dependency population.

Knowledge of the various treatment mediums for addiction treatment as well as state and local resources.

Must have and maintain State of West Virginia Peer Recovery Support Specialist certification.

Knowledge of agency, state, and federal guidelines.

Ability to recognize and comply with client’s rights.

Ability to comply with Valley Healthcare and the divisional departments safety protocols and procedures.

Ability to read, write, and speak the English language.
PREFERRED QUALIFICATIONS:

Prior experience in programs related to behavioral services and treatment diversion intervention.

Experience in working with diverse populations.

Proven ability in professional public speaking.
