We are seeking a Home Health Occupational Therapist (OT) who values flexibility, efficient workflows, and a supportive team that understands what home health is actually like. This role is designed to give you control over your schedule, your coverage area, and your workload — without unnecessary administrative burden.

Duties
- Evaluate patients’ functional abilities and develop individualized plans of care focused on ADLs and IADLs
- Provide skilled occupational therapy interventions to improve safety, mobility, and independence in the home
- Address areas such as dressing, grooming, toileting, transfers, and functional mobility
- Assess home environments and recommend adaptive equipment or safety modifications as appropriate
- Educate patients and caregivers on strategies to promote independence and reduce fall risk
- Communicate patient progress and clinical changes with the interdisciplinary care team and physician as needed
- Maintain accurate, timely electronic documentation in compliance with home health and regulatory standards
Qualifications
- Active and unrestricted California Occupational Therapist license
- Graduate of an accredited Occupational Therapy program
- Home health experience preferred, but not required
- Strong understanding of ADLs, functional mobility, and patient safety
- Excellent communication and time-management skills
- Ability to work independently in a home-based setting
- Reliable transportation and valid driver’s license
Position Details
- Pay: $85 – $135 per visit
- Schedule: PRN / Per diem / Contract
- Coverage Area: Your preference coverage in Ventura County
- Work Location: In person (home health)
