Job Opportunity: PRN Registered Nurse - Supervisory Role (RN, LPN, NP)

YFI Homecare Solutions, LLC is seeking a dedicated and detail-oriented PRN Registered Nurse to join our team. We are a in-home care agency committed to providing high-quality in-home respite care through the CLTC waiver program in partnership with SCDHHS Medicaid.
Position Overview
As a Supervising Nurse, you will play a critical role within our organization by conducting Supervisory Visits and ensuring compliance with CLTC guidelines. Your responsibilities will include completing initial assessments for patients entering our programs, conducting periodic supervisory visits, and maintaining accurate documentation of all evaluations. Please note that this position does not require you to provide direct care.
Key Responsibilities
· Perform an Initial Supervisory Visit prior to the start of services upon authorization.
· Conduct a 30-day Supervisory Visit from the initial service date.
· Perform subsequent Supervisory Visits every four months, scheduled on the last day of the month. If the participant is unavailable on the last day, the visit must be completed within five days to remain compliant.
· Ensure all assessments are conducted in person and documented in compliance with CLTC guidelines.
· Complete a new Nurse Supervisory Visit if a client is unavailable for more than 60 days after the initial visit.

Position Details
· This is a 1099 contractor position, paid on a fee-for-service basis.
· All evaluations and visits must align with the compliance requirements of the CLTC waiver program.
Qualifications
· Active Registered Nurse (RN) license in the state of South Carolina
· Strong attention to detail and ability to adhere to compliance guidelines
· Excellent communication and documentation skills
· Reliable transportation for in-home visits
