M&L Prodigy Healthcare Inc. is a MassHealth Adult Foster Care (AFC) provider based in Woburn, MA. Our program supports older adults and adults with disabilities who live safely at home with the support of a dedicated live-in caregiver. Our multidisciplinary team -- registered nurse, care management, and program leadership -- partners with caregivers to keep members healthy, safe, and independent in their communities.
About the Role
We are hiring a per diem RN or LPN to conduct member home visits in our southern service area -- Brockton, Quincy, Randolph, Lakeville, and surrounding towns. You will visit members in the home where they receive care, monitor their health status, and complete a nursing progress note for each visit, following the program's established visit schedule. Visits are brief (typically 20–30 minutes), scheduled flexibly on weekdays, and grouped geographically to minimize driving — roughly 3–8 hours per week depending on program census. This is a focused role: visits and documentation are yours, while clinical assessments, care plans, and program administration are handled by our core clinical team. LPNs work under the direction and supervision of our agency's Registered Nurse.
What You'll Do
- Conduct scheduled on-site visits with members in AFC qualified settings (the caregiver's home)
- Monitor and document each member's health status, care delivery, and home environment
- Review the AFC caregiver log during each visit
- Complete a nursing progress note for every visit within the required timeframes
- Promptly report changes in member condition or any concerns to the lead RN and Program Director
- Conduct occasional additional visits for new admissions or changes in condition, as assigned
- Communicate warmly and professionally with members, caregivers, and families
- Complete agency orientation and required annual training
Requirements
- Current Massachusetts RN or LPN license in good standing with the Board of Registration in Nursing
- For RNs: at least two years of recent experience in the direct care of elders or adults with disabilities (per 130 CMR 408.433); comparable experience strongly preferred for LPNs
- Valid driver's license, reliable transportation, and willingness to travel to member homes in the Brockton/Quincy/Randolph/Lakeville area
- Ability to pass CORI, SORI, and OIG background screenings
- Tuberculosis screening within the past 12 months (or willingness to complete before start)
- Strong documentation skills and comfort with electronic records
Preferred
- Prior experience in Adult Foster Care, GAFC, home health, or other community-based services
- Bilingual ability (Spanish, Haitian Creole, Portuguese, or other languages) is a plus
Pay & Schedule
- $43–$55 per Visit depending on licensure and experience (LPNs typically in the lower band, RNs in the upper band), plus mileage reimbursement
- Per diem / part-time with flexible weekday daytime scheduling
- Ideal supplemental role for a nurse seeking meaningful, low-volume field work close to home
