Job Summary

Duties
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
- Monitor and observe patient health status, including vital signs and overall well-being
- Support medication administration following proper protocols and documentation standards
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively
- Assist with feeding, ensuring dietary restrictions are followed
- Perform light housekeeping duties including cleaning and laundry to maintain a safe and comfortable environment
- Document patient progress and communicate updates to healthcare teams promptly
- Offer emotional support and companionship to foster a positive home environment
Requirements
- Prior experience in home health care preferred
- Strong knowledge of patient care procedures
- Certification in CPR and first aid required; additional certifications such as hospice care or dementia training are a plus
- Experience working with individuals with developmental disabilities or long-term care needs preferred
- Ability to operate medical equipment like Hoyer lifts and perform catheter care safely
- Familiarity with EMR/EHR systems for accurate documentation of patient information
- Excellent communication skills and compassionate demeanor
- Ability to work independently while following healthcare protocols and safety guidelines
- Valid driver’s license and reliable transportation are often necessary for home visits
