Position Overview:
We are seeking an experienced Registered Nurse (RN) – Infectious Disease & Special Programs to support outpatient and community healthcare services across multiple locations in San Diego County.
This role is ideal for an RN with outpatient, community health, infectious disease, HIV/STI, behavioral health, or population health experience.
Key Responsibilities:
- Provide patient-centered nursing care in accordance with established clinical standards and organizational policies.
- Perform in-person and telephone triage to assess patient needs and determine appropriate care.
- Conduct RN intake processes for patients establishing care.
- Schedule new patients and facilitate timely access to appropriate healthcare services.
- Participate in co-visits to support comprehensive patient care and improve access to services.
- Apply clinical judgment and problem-solving techniques to address patient needs.
- Provide education related to STI, HIV, infectious disease transmission, prevention, and health promotion.
- Collaborate with physicians, advanced practice providers, nurses, behavioral health teams, and other internal departments.
- Coordinate with external healthcare and community organizations as needed.
- Support patients with complex healthcare needs, including individuals receiving care for mental health conditions and/or substance use disorders.
- Maintain accurate and timely clinical documentation.
- Participate in meetings, quality improvement activities, and organizational initiatives.
- Maintain readiness for audits and support compliance with applicable clinical and regulatory standards.
- Promote safe, high-quality, equitable, and cost-effective patient care.
- Support population health management initiatives designed to improve health outcomes within the communities served.
Required Qualifications:
- Active California Registered Nurse (RN) License
- 2+ years of Registered Nurse experience
- Previous outpatient or community healthcare experience
- Knowledge of STI/HIV care, infectious disease transmission, and prevention
- Experience working with patients with mental health conditions and/or substance use disorders
- Experience in population health, care coordination, triage, or community health settings
- Basic Life Support (BLS) certification through the American Heart Association
- Associate Degree in Nursing (ADN)
- High School Diploma or GED
- Bilingual in English and Spanish
Pay: $55.00 - $65.00 per hour
Expected hours: 40.0 per week
