Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
- Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Administer medications and perform catheter care following home healthcare regulations and medical terminology standards.
- Support clients with dementia or Alzheimer’s disease through specialized memory care techniques and compassionate engagement.
- Provide companionship and emotional support while maintaining patient confidentiality in accordance with HIPAA guidelines.
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients with mobility challenges or disabilities.
- Document medical records and care activities thoroughly to ensure compliance with home health agency standards and regulatory requirements.
Experience
- Prior experience working in home care setting is highly preferred.
- Demonstrated knowledge of home health care assistance, geriatrics, dementia care, and end-of-life care practices.
- Experience working with individuals with Alzheimer's and Dementia
- Strong understanding of home & community care regulations to ensure all services are delivered safely and compliantly.
