We are seeking a compassionate, dependable, and experienced Registered Nurse (RN) to join our home health care team. The RN will provide skilled nursing services to patients in the comfort of their homes while coordinating care with physicians, caregivers, and other healthcare professionals.
The ideal candidate is organized, clinically knowledgeable, comfortable working independently, and committed to providing high-quality, patient-centered care.
Responsibilities
- Perform skilled nursing visits in patients’ homes
- Complete comprehensive nursing assessments and reassessments
- Develop, implement, and update individualized plans of care
- Monitor patients’ conditions and identify changes requiring intervention
- Administer medications and treatments as ordered
- Provide wound care and other skilled nursing procedures
- Educate patients and caregivers regarding medications, diagnoses, treatments, and disease management
- Coordinate patient care with physicians, LPNs, therapists, caregivers, and other healthcare professionals
- Supervise and provide clinical direction to LPNs and other assigned staff as appropriate
- Communicate changes in patient condition to physicians and the appropriate members of the care team
- Complete accurate and timely clinical documentation
- Assist with admissions, recertifications, resumptions of care, and discharges as assigned
- Maintain compliance with agency policies and applicable state and federal requirements
- Promote patient safety, independence, dignity, and quality of life
Qualifications
- Current and unrestricted Registered Nurse (RN) license in Nevada
- Graduate of an accredited nursing program
- Current CPR/BLS certification
- Valid driver’s license and reliable transportation for home visits
- Strong clinical assessment and critical-thinking skills
- Excellent communication, organization, and documentation skills
- Ability to work independently in a home health setting
- Home health, skilled nursing, or related nursing experience preferred
Job Type: Full-time
Pay: $45.00 - $60.00 per hour
