MDS Coordinator (RN/LPN)
Calvert City Nursing & Rehabilitation, a respected independent non-profit skilled nursing facility, is seeking an experienced and dedicated MDS Coordinator to join our exceptional team. This is a full-time, on-site position offering the opportunity to make a meaningful impact on resident care while working in a collaborative and supportive environment.
Position Summary
The MDS Coordinator is responsible for coordinating and completing accurate and timely resident assessments in accordance with federal and state regulations. This role works closely with the interdisciplinary team to ensure compliance, support quality outcomes, and maximize clinical reimbursement through accurate documentation and assessment practices.
Essential Responsibilities
- Coordinate and complete accurate, timely Minimum Data Set (MDS) assessments in compliance with federal and state regulations.
- Manage all aspects of the RAI process, including assessment scheduling, completion, submission, and interdisciplinary coordination.
- Collaborate with nursing, therapy, social services, dietary, and other departments to ensure comprehensive resident assessments.
- Review resident records and clinical documentation to ensure assessment accuracy and regulatory compliance.
- Monitor documentation practices to support appropriate reimbursement and quality measures.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities and initiatives.
- Educate and support interdisciplinary team members regarding MDS requirements, documentation standards, and regulatory updates.
- Identify opportunities to improve clinical documentation, compliance, and resident outcomes.
- Maintain compliance with facility policies, professional nursing standards, and all regulatory requirements.
- Participate in daily clinical meetings and weekly care plan meetings.
Qualifications
- Current, active, unencumbered Kentucky nursing license (RN or LPN) or eligibility through a multistate compact license.
- Previous MDS Coordinator experience strongly preferred.
- Strong knowledge of the RAI process, MDS 3.0 requirements, and skilled nursing facility regulations.
- Long-term care experience preferred.
- Current CPR certification.
- Excellent written, verbal, and interpersonal communication skills.
- Ability to read, interpret, and apply regulatory requirements.
- Demonstrated leadership abilities with experience coaching and supporting staff.
- Professional, compassionate, and resident-centered approach to care.
- Ability to meet the physical requirements of the position.
