Family Nurse Practitioner (FNP)
PCP for Life | Houston, TX (Multiple Locations) | $120,000–$155,000/yr | Rotating Schedule – Up to 4 Days Off Per Week
---
Most outpatient FNP roles offer you a panel, a schedule, and a charting system that slows you down. This one is built differently.
At PCP for Life, you'll practice with real clinical autonomy, a dedicated MA team supporting you per provider, a fully implemented eClinicalWorks system, and a rotating schedule that gives you a 4-day weekend every other week. No nights. No hospital rounding. No on-call.
We've been caring for Houston's diverse communities since 1992 — eight clinics strong, with new locations opening in Willis and Fulshear. Our patient population is multicultural, Spanish-speaking, and deeply loyal. If you want to build lasting relationships with patients across the full spectrum of primary care, this is where that happens.

THE SCHEDULE — WORTH LEADING WITH
FNP burnout is real, and schedule is one of the biggest drivers. Here's what ours actually looks like:
- Week 1 — 3 days on, 4 days off: 2 weekday shifts (12 hours each) + 1 weekend shift (10 hours). Then 4 full days off.
- Week 2 — 4 days on, 3 days off: 3 weekday shifts (12 hours each) + 1 weekend shift (10 hours). Then 3 days off.
- The result: A 4-day weekend every other week. Outpatient primary care with real recovery time built in — not promised someday.

WHAT YOU'LL DO
This is a full-scope outpatient primary care role. You'll see 30–40 patients per provider each day across a broad mix of acute, preventive, and chronic disease visits — the kind of clinical variety that keeps the work meaningful.
- Patient Assessment & Treatment – Conduct comprehensive evaluations, develop and execute treatment plans, and manage a wide range of acute and chronic conditions across all ages.
- Chronic Disease Management – Manage complex chronic conditions including diabetes, hypertension, and heart disease with a focus on outcomes and patient education.
- Preventive Care – Deliver annual wellness visits, screenings, and health promotion counseling in line with current clinical guidelines.
- Procedures – Perform venipuncture and other in-office procedures within scope.
- Care Coordination – Collaborate with your MA team, referring providers, and our CCM Coordinator team to ensure continuity between visits.
- Documentation – Chart accurately and efficiently in eClinicalWorks. Our system is optimized — we expect documentation to stay current, and we've built the workflow to support it.
