We are actively seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join a Skilled Nursing Facility in Novato, CA through Therapy HCR. Key Responsibilities
- Provide skilled physical therapy treatment under the direction and supervision of a licensed Physical Therapist.
- Assist patients with therapeutic exercises, mobility training, balance activities, strengthening, and functional activities.
- Monitor and document patient progress and response to treatment.
- Communicate patient progress and concerns to the supervising Physical Therapist and interdisciplinary team.
- Educate patients and caregivers on treatment techniques, safety, and prescribed home exercise programs.
- Maintain accurate and timely documentation in compliance with facility and regulatory standards.
- Participate in care planning and support patient-centered rehabilitation goals.
Qualifications
- Active California Physical Therapist Assistant (PTA) License.
- Graduate of an accredited Physical Therapist Assistant program.
- SNF experience preferred, but not required.
- Strong clinical, communication, and documentation skills.
- Ability to work effectively in a patient-centered, team-oriented environment.
