Nevada Senior Services is recruiting a Care Transitions Specialist (Licensed Social Worker)to join their team. The Social Worker acts as a single point of entry for long-term support for older adults, people with disabilities (of any age), family members and care providers. Nevada Senior Services helps individuals of all ages and income levels obtain the services and support they need by establishing one-on-one assistance through a single organization. This position works to enhance independent living by providing access to needed services and reducing the stress of navigating benefits.
DUTIES AND RESPONSIBILITIES:
- Provide comprehensive, interactive decision support process that examines a consumer’s needs, preferences, values and strengths, which results in a person-centered service plan.
- Inform and educate people about their long-term care options; assist in connecting them to programs, service and public benefits and completing applications for services.
- Provides advocacy and empowers consumers to make informed long-term decisions as a means to maintain independence.
Uses appropriate interview techniques, such as active listening over the phone, or in person.
- Gathers sufficient information to accurately identify areas of concern, problems and/or needs.
- Searches through resource databases, as well as other written and computer-based information resources to evaluate and suggest potential programs and services.
PROFESSIONAL REQUIREMENTS:
- Requires an active Social Work license in the state of Nevada.
- Possess excellent oral and written communication as well as management skills.
- Enthusiastic, flexible and display ability to work cooperatively with staff at all levels.
- Detail-oriented, task-oriented, and non-judgmental with a clear sense of personal boundaries.
- Proficiency in computers and software including Microsoft Office, ability to maintain accurate and complete documentation in a web-based record system.
- Resourceful, takes initiative and has strong leadership qualities. Highly organized, ability to prioritize and work in crisis.
MINIMUM QUALIFICATIONS:
- Bachelor’s degree required from an accredited college or university.
- Must have a background in facilitating Support Groups.
- History in working with the senior population and individuals with chronic diseases and people with disabilities.
- Must complete fingerprint check as a condition of employment.
- Must pass a two-step TB test.
WORKING CONDITIONS:
- Fieldwork requires travel to clients in a variety of settings, homes, adult day care facilities, etc. of varying levels of cleanliness and repair. Fieldwork requires driving personal vehicle.
- Contact with the public in home or office environments may risk exposure to contagious diseases and/or contact with domestic animals. .
- Requires ability to work under stress as it pertains to the duties and responsibilities.
