Sparx Support Services is seeking compassionate, organized, and motivated Adult Care Managers to join our growing Care Management team.
Sparx Support Services is a Care Management Agency helping adults connect with the healthcare, behavioral health, and community-based services they need to improve their overall health and quality of life.
Position Details
- Position: Adult Care Manager
- Schedule: Part-Time
- Hours: Up to 65 hours per month
- Pay: $40.00 per hour
- Location: Remote
- Program: Adult Health Home Care Management
Responsibilities
As an Adult Care Manager, you will:
- Manage and coordinate services for an assigned caseload of adult members.
- Develop individualized care plans based on each member’s needs and goals.
- Connect members to medical, behavioral health, mental health, substance use, and community-based services.
- Assist members with accessing benefits, Medicaid services, home care, housing resources, transportation, and other social supports.
- Coordinate with physicians, behavioral health providers, hospitals, specialists, and community organizations.
- Conduct regular member outreach and follow-up.
- Complete required documentation accurately and on time.
- Help members navigate the healthcare system and overcome barriers to receiving care.
- Participate in required trainings and supervision.
Qualifications
- Strong communication, organization, and interpersonal skills.
- Comfortable working independently in a remote environment.
- Ability to manage multiple members and maintain timely documentation.
- Experience in care management, healthcare, behavioral health, social services, or community-based services is preferred.
- Familiarity with New York Medicaid and Health Home services is a plus.
- Bilingual candidates are strongly encouraged to apply.
