About the Role:
Cypress Coast Care LLC is looking for a dedicated and compassionate Home Care RN Supervisor to join our growing team. This is an exciting opportunity to lead and inspire a team of caregivers while making a meaningful difference in the lives of patients receiving care in their homes. Responsibilities:
- Supervise and support home care staff, including CNAs and home health aides, to ensure quality patient care
- Conduct patient assessments, develop individualized care plans, and monitor ongoing patient progress
- Perform in-home nursing visits and clinical evaluations as needed
- Ensure compliance with state, federal, and agency regulations governing home health care
- Coordinate with physicians, therapists, and other healthcare providers to deliver comprehensive patient care
- Provide staff training, mentorship, and performance evaluations
- Maintain accurate and timely clinical documentation and patient records
Requirements:
- Active RN license in the state of North Carolina (required)
- Minimum 2 years of nursing experience, preferably in home health or community care
- Prior supervisory or leadership experience in a healthcare setting preferred
- Strong knowledge of home health regulations, Medicare/Medicaid guidelines, and care plan development
- Excellent communication, organizational, and critical thinking skills
- Valid driver's license and reliable transportation for home visits
- Compassionate, patient-focused demeanor with a commitment to quality care
