Job Location: Lewisburg, WV and surrounding areas
Position Summary:
Provides a comprehensive social service/advocacy program for assigned patients and/or family members as required or requested.
Serves as HospiceCare liaison within the social service network all counties served by HospiceCare.
Performs other related duties as required.
Position Qualifications:
Graduate of an accredited school of social work.
BSW required, MSW preferred.
Current unrestricted WV social work license.
Grief counseling experience preferred.
Experience in home health/hospice preferred.
Contingent upon successful completion of drug screen, background check, and other screenings as applicable.
Work Environment:
Office and healthcare facility setting.
Frequent interaction with patients, families, staff, and community resources.
May involve exposure to stressful or emotional situations.
Regular use of computers, phones, and other office equipment.
Physical Requirements:
Ability to sit, stand, and walk throughout the workday.
Ability to communicate effectively.
Ability to occasionally lift and carry materials up to 20 pounds.
