REQUIREMENTS:
EDUCATION:
Bachelor's or Master’s degree in social work preferred or a related Health & Human Services field from an accredited university or college. QIDP preferred, not required.
EXPERIENCE:
Minimum of one year of experience in working with Developmentally Disabled individuals.
CERTIFICATION/LICENSURE:
Current requisite professional licensure. Meet requirements of Medicaid for a Qualified Intellectual Disabilities Professional (Defined as: An individual with specialized training or 1 year experience in treating or working with a person who has intellectual disability; and is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, registered nurse, registered dietician, therapeutic recreation therapist, or a licensed or limited licensed professional counselor. An individual with a bachelor’s degree in a human services field who was hired prior to January 1, 2008 and performed in the role prior to January 1, 2008 would also qualify).
SUMMARY OF RESPONSIBILITY:
Shiawassee Health & Wellness, an innovative community mental health organization, is seeking a full-time Case Coordinator to provide consumer assessment, reassessment, and person-centered planning, coordination of care, and linkage of agency and inter-agency resources and advocacy services. The Case Coordinator is the primary provider when a consumer is in need of multiple services. He/she is responsible for the content of the Person-Centered Plan (PCP) so that regulatory/Medicaid compliance is assured and the PCP reflects consumer strengths, desires, dreams, needs and goals. The Case Coordinator ensures that services are provided in the least restrictive setting and that the Person-Centered Plan allows the consumer to access community-based services. This Coordinator reports to the Deputy Director of Habilitative Services.
