Position Overview
We are seeking an experienced RN Case Manager to join an acute hospital team for a 13-week contract with the possibility of extension.
The ideal candidate will have a strong background in acute hospital Case Management, Care Coordination, Utilization Review, and Discharge Planning, along with recent hands-on experience in an acute-care setting.
Requirements
- Minimum 2 years of acute hospital Case Management experience
- Acute hospital Case Management experience within the last year is required
- Strong experience with Care Coordination and Discharge Planning
- Epic experience required
