Director of RN Care Management
Location: Mission Hills, CA
Salary: $250,000 - $325,000 Base Salary

SmartXhire is partnering with a leading acute care hospital in Mission Hills, California, to hire a Director of RN Care Management.
This is an executive-level opportunity for an experienced nurse leader who wants to influence care delivery, improve patient outcomes, and lead hospital-wide care management strategy.
The Director of RN Care Management will oversee key functions including Case Management, Utilization Management, Discharge Planning, Care Transitions, and interdisciplinary care coordination while partnering with physicians, clinical leaders, and hospital executives.
Key Responsibilities
- Provide strategic leadership for hospital Care Management operations
- Oversee RN Case Management, Utilization Management, and discharge planning functions
- Lead and develop care management teams to achieve clinical and operational goals
- Improve patient flow, length of stay, readmission outcomes, and care transitions
- Collaborate with physicians, nursing leadership, social work, and interdisciplinary teams
- Drive quality improvement initiatives and operational performance metrics
- Partner with executive leadership on care management strategy and initiatives
Qualifications
Required:
- Active California Registered Nurse (RN) License
- Bachelor of Science in Nursing (BSN) required
- Master's degree in a health-related or science field
- Minimum 2 years of acute care case management experience
- Minimum 2 years of leadership experience managing care management or case management teams
Preferred:
- Experience leading care management operations within an acute care hospital environment
- Strong understanding of utilization management, discharge planning, and care coordination
- Proven ability to collaborate with physicians and executive leadership
