Job Overview
We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our healthcare team. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans
- Administer medications and injections accurately following physician orders, including IV infusions and other specialized treatments
- Monitor patients’ conditions closely, recognizing signs of acute pain or deterioration and responding promptly with appropriate interventions
- Assist with diagnostic evaluations such as specimen collection, medical imaging preparation, and catheterization procedures
- Maintain detailed and accurate medical documentation within Electronic Health Record (EHR) systems while ensuring compliance with HIPAA and confidentiality policies
- Provide patient education on medication management, disease processes such as dementia or Alzheimer's care, and post-discharge instructions
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with complex needs including geriatrics, pediatrics, or those requiring hospice or palliative care
Skills
- Valid LVN license with current state certification and licensing requirements met
- Extensive experience working with diverse patient populations including geriatric patients, individuals with developmental disabilities, or those requiring memory or dementia care
- Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for efficient documentation and communication
- Strong knowledge of medical terminology, anatomy, physiology, and pharmacology relevant to acute and chronic care settings
- Skilled in IV insertion, catheter care, venipuncture (phlebotomy), medication administration, and sterile technique
- Experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable
- Ability to perform patient assessments accurately in fast-paced environments like emergency rooms or urgent care clinics
- Knowledge of long-term care regulations and best practices for skilled nursing facilities and assisted living communities
- Excellent communication skills to educate patients effectively and collaborate with healthcare teams on discharge planning or case management strategies
- Demonstrated ability to work independently while adhering to clinical confidentiality policies and safety protocols
