Overview
Bright Start PT & PTA, PLLC is a NYS Early Intervention agency specializing in providing Early Intervention services to children from birth to three (3) years of age in all 5 NYC boroughs as well as Nassau and Suffolk Counties on Long Island. We are currently seeking a dedicated Service Coordinator to assist parents in developing plans and strategies to meet their children's and family's needs.

Duties
- to ensure that parents are informed about the Early Intervention Program (EIP) and their rights under the program
- to coordinate all aspects of the screening and evaluation process to determine a child's eligibility for the Early Intervention Program.
- to discuss the Individual Family Service Plan ( IFSP) process with the parents of children determined to be eligible for early intervention services
- to participate in the meetings to develop the IFSPs
- to secure and arrange for providers to deliver Early Intervention Program services
- to monitor the delivery of early intervention services in accordance with the IFSP
- to maintain proper documentation on families and children in accordance with EI requirements
Requirements
- Bachelor's or Master's degree in a health or human service field including psychology, social work, special education, early childhood development, or other related licensed healthcare professional
- One year of service coordination experience and an Associate's Degree in a health or human service field
- Two years of experience in service coordination activities
- Strong organizational and communication skills, with the ability to effectively manage multiple tasks and prioritize responsibilities
- Ability to work independently and as part of a team
Pay: $55,000.00 - $75,000.00 per year
