We’re currently seeking a dedicated Full-Time RN Care Coordinator to become part of our caring and mission-driven community. This position is Monday through Friday, 8:00 AM to 4:00 PM, with on-call responsibilities as needed.
The RN Care Coordinator works closely with residents, families, and healthcare providers to coordinate care, provide education, and connect residents with resources that support healthy, independent living.

Develop and implement the community's Care Coordination program, integrating best practices from hospital-based navigation models.
Identify and engage high-risk residents, serving as the primary clinical resource for Residential Living.
Create individualized care plans in collaboration with physicians and the healthcare team.
Conduct post-hospital and emergency room follow-ups, including review of discharge instructions, appointment scheduling, and medication reconciliation.
Provide basic clinical services such as blood pressure checks, dressing changes, and glucose monitoring; address resident questions and concerns.
Serve as liaison between residents and healthcare providers, including specialists such as dermatologists, neurologists, and podiatrists.
Guide residents and families through the healthcare system and connect them with community resources and support services.
Advocate for residents and families to ensure access to appropriate care and services.
Respond to emergency alerts within the community and follow up with residents who are unwell.
Conduct comprehensive nursing assessments to identify needs and support disease self-management.
Participate in interdisciplinary care planning meetings and contribute to goal setting with residents and families.
Track and document all resident interactions, outcomes, and quality metrics.
Coordinate regularly with hospital navigation and community health teams.
May serve as the Employee Wellness Nurse, coordinating pre-employment physical and drug test results, managing employee health records, administering workers' compensation processes, and organizing annual flu shot clinics for employees and residents.
Participate in continuing education and serve on internal and external committees as needed.
Perform additional duties as assigned.

What You’ll Need as a RN Care Coordinator:
Graduate of an accredited nursing program with an RN degree; BSN preferred.
Current, active Registered Nurse license in the State of Maryland.
Advanced nursing certifications are a plus.
Minimum of 2 years of RN experience in acute care, long-term care, or community outpatient settings; experience in long-term care or assisted living preferred. Healthcare Provider CPR certification required.
