RN Assessment Coordinator (RNAC) - Southmont
Washington, PA – Full Time; Hourly, Non-exempt

The RN Assessment Coordinator is responsible for conducting thorough assessments of residents’ medical and care needs, developing care plans, and ensuring accurate documentation for reimbursement purposes. This position plays a lead role in coordinating care delivery, monitoring resident progress, and maintaining compliance with regulatory requirements. As a key support member of the skilled nursing leadership team, candidates will be required to exhibit excellent customer service behaviors and interactions through effective communication with team members, residents, and family members.

Requirements for the RN Assessment Coordinator:
-
Possess a valid RN license issued by the State of Pennsylvania and maintain current CPR certification, per policy.

Have a minimum of 1 year of long-term care experience; supervisory experience in a skilled nursing environment is preferred.

At least 2 years of experience with completing MDS and facilitating interdisciplinary care planning.

Strong interpersonal skills to support, advise, coach, and influence others;

Exhibit proficiency with MDS software as well as PC and Microsoft Office applications.
