Hospice RN Case Manager - Olmsted Falls Territory
$80,000 - $85,000 per year
Northeast Ohio Hospice is looking for a full-time RN Case Manager to join our team.
This position serves patients primarily in the Westlake and Olmsted Falls area. Our nurses typically manage a caseload of 12-15 patients, most of whom reside in assisted living or skilled nursing facilities. We believe this allows our nurses to spend less time driving and more time building relationships with patients, families, and facility staff.
As a Hospice RN Case Manager, you will be responsible for coordinating and providing nursing care to hospice patients, developing and updating plans of care, educating patients and families, and collaborating with members of the interdisciplinary team.
Responsibilities:
- Complete nursing assessments and routine visits.
- Manage an assigned caseload of hospice patients.
- Develop, implement, and update individualized plans of care.
- Provide symptom management and patient/family education.
- Coordinate care with physicians, hospice aides, social workers, chaplains, and facility staff.
- Complete admissions and other visits as needed.
- Maintain timely and accurate documentation.
- Participate in on-call rotation as needed.
Qualifications:
- Current Ohio RN license.
- Minimum one year of hospice experience required.
- Strong assessment and communication skills.
- Ability to work independently and as part of a team.
- Reliable transportation, valid driver's license, and proof of auto insurance.
